PURPOSE: Assesses, plans, implements, evaluates and supervises individual patient care on a nursing unit according to unit policies and procedu! res. Accepts responsibility for directing implementation of patient care. May be required to work on other nursing units according to distribution of staff and patients. JOB RELATIONSHIPS: A. Responsible to Clinical Director or designee. B. Workers Supervised: Directs positions including, but not limited to, RN's, LPN's, unlicensed assistive personnel, unit secretaries and students in collaboration with instructors. C. Inter-relationships: Works closely with all members of the nursing staff, medical staff, patients and their families and other ancillary personnel. ESSENTIAL JOB FUNCTIONS: 1. Assesses and analyzes the health status (physical assessment and admission history) of patients on assigned unit in a comprehensive manner. 1:1 Initial observation is made on arrival specific to patient needs. a. Completes required assessments within established unit standards. 1:2 Initial observation is made on arrival specific to patient needs. a. Completes Admission/History form with! in established unit standard. 1:3 Ongoing assessment is perfor! med based on patient condition. a. Routine physical assessment is completed according to unit standards. 1:4 Promptly identifies changes in patient condition and reports to physician and/or healthcare professionals. 2. Plans patient care on assigned unit in collaboration with the patient, family and health care team as demonstrated through a record review. 2:1 Initiates nursing plan of care per unit standards. 2:2 Nursing Plan of care reflects the specific physical, psychosocial and safety needs of each patient. 2:3 Nursing Plan of care reflects measurable short and long term goals/outcomes according to patient assessment. 2:4 Nursing Plan of care is updated to reflect changes in the patient condition. 2:5 Nursing Plan of care reflects appropriate designation of responsibilities, (Social Services., PT, Dietary). 2:6 Nursing Plan of care includes discharge planning which is initiated on admission and completed prior to discharge. 3. Directs and implements patient care activi! ties on assigned unit to insure the delivery of quality care. 3:1 Communicates the on-going plan of care to other members of the health care team, (i.e., walking rounds, patient care plan, verbally during the shift, unit-specific flow sheet). 3:2 Makes appropriate staff assignments each shift following unit standards. Associate exceeds expectation by routinely assuming resource duties in the absence of the Shift Manager. 3:3 Communicates/documents patient care given including assessments, planned interventions, patient response, and follow-up actions. 3:4 Implements and documents appropriate discharge planning and instructions/teaching according to Midwest Regional Medical Center policy. 3:5 Transcribes and directs the implementation of physician orders. 3:6 Administers medication according to hospital policy and procedure. 3:7 Reports all shift incidents, accidents, or errors to Clinical Director or designee and adheres to medication and incident variance reporting. 3:8 In! sures unit standards of care and measures compliance through Performanc! e Improvement procedures. 4. Evaluates and documents patient/family response and outcome to nursing care. 4:1 Evaluates the patients response to nursing interventions as demonstrated through complete documentation. 4:2 Revises nursing plan of care to keep goals current. 4:3 Identifies issues related to patient outcomes and initiates corrective action process. 5. Implements and documents patient teaching and patient outcomes. 5:1 Formulates an individualized teaching plan based on ongoing assessments of the patient's needs and educational level. 5:2 Includes the patient and appropriate family members in formulation of teaching plan. 5:3 Reviews and reinforces the teaching plan with patient and family. 5:4 Documents teaching plan and patient/families understanding of the plan according to unit standards. 6. Accountable and responsible for individual nursing practice by annually completing unit specific competencies and checklist. 7. Demonstrates growth as a professional nurse! as evidenced by one or more of the following: a. Acts as preceptor for orientee/students. b. Certified in nursing specialty. c. Actively attends college courses in nursing related field. d. Participates in community health related programs/activities. e. Membership in a professional nursing organization. f. Serves on hospital/unit based nursing services committee or Task Forces as needed. g. Initiates or teaches a unit based program or conference. h. Other 8. Performs patient assessment demonstrating awareness of growth and development for all age groups. Demonstrates knowledge of age specific assessment, psychosocial levels and developmental patterns. Provides for age appropriate stimulation and is knowledgeable regarding learning needs specific to the patient population. Demonstrates knowledge for the following patient populations: birth-1 yr; 1-11 yrs; 12-17 yrs; 18-64 yrs; 65+ yrs through completion of age related competencies. Observation of skills and abilities relat! ed to specific patient population. 8:1 Knowledge of growth and age deve! lopment. 8:2 Ability to assess age specific data. 8:3 Ability to interpret age specific data. 8:4 Ability to provide specific care. 8:5 Possess communication skills necessary to interpret age specific response to treatment. 8:6 Ability to involve family and significant other in decision making related to plan of care. 8:7 Understands the range of treatment needed by specific age group. 9. Reliable attendance is an essential function of this position. 10. Adhere to all hospital safety policies and procedures.

Requirements

Graduate of an accredited school of nursing with a current Oklahoma license.

Bachelors Degree is required.

ACLS mandatory.

CPR mandatory.

CCRN recommended.

Mandt Training encouraged.

Port Certification mandatory.
