The Quality Manager/Patient care coordinator-RN will be responsible in oversight and management of the quality program. The quality care manager will provid! e leadership and supervision to the patient health care coordinators and medical assistants. Is responsible for assisting physician led care teams to achieve identified quality patient care goals. Expected to perform as a team member to support and enhance the system-wide quality improvement program. Responsible for collecting, organizing, and measuring clinical indicators and data. Expected to assist in the development and implementation of tools, guidelines, and best practices to improve efficiency, quality of care, and patient outcomes by clincal teams and patient health care coordinators. Provide positive staff support and education and act as a resource to clinical staff and administration.
This position is located at 1145 S Utica in Tulsa, OK.
Requirements:
High school diploma or GED and an Associate of Applied Science in Nursing required. Current OK RN license and more than 5 years nursing experience. Prefer experience in geriatrics/med. ! surg. and at least on year physician office experience (adult ! medicine). Prefer at least 2 years program management/supervisory experience (adult/family medicine). Quality Improvement, Case Management, Utilization Review, Risk Management/Safety/Patient Safety, and Employee Health experience a plus. Will oversee and/or assist with building and maintaining patient registries to identify gaps in care and lead to better healthcare outcomes for patients. Will collaborate with multi-disciplinary teams at medical practice level and be a leader in change. Will assist clinical care teams by providing tools and information to facilitate individualized care for each patient, delivery of timely preventive care and coordination of care across the medical neighborhood. The patient health coordinator will have excellent written and verbal communication skills. Will be proficient in computer keyboarding and software applications including MS Word, MS Excel, MS Powerpoint. Will be proficient with the electronic patient health record (NextGen), IDX, an! d population health management tools. Will interpret and convey complex information, using critical thinking skills. Will have a knowledge of patient care medical home requirements, preventative health maintenance, and evidence based clinical guidelines, quality improvement processes, and case management and regulatory guidelines. Promotes and supports the implementation and monitoring of system policies and guidelines, OSHA regulations, and other state and federal laws or regulations. Should have knowledge of ambulatory care, quality improvement, patient safety, staff development, environment of care, and employee health.
