South! western Regional Medical Center is currently seeking a Clinica! l Documentation Specialist to perform concurrent reviews of medical records to identify opportunities for documentation improvement. This position will generate written queries in cases where additional documentation is needed to support the severity of the patient's condition, specificity of diagnosis and treatment performed, and to ensure that the documentation ensures accurate diagnostic and procedural coding. Responsibilities may also include the education of members of the patient empowered care (PEC) team regarding documentation guidelines, including attending physicians, mid-level practitioners, allied health practitioners, nursing, and case management. The CDS will act as the liaison between the coding professionals, health record analysts and the medical staff. Use of communication and documentation skills, coupled with clinical knowledge in the management of oncology patients is necessary to continuously improve the enterprise documentation efforts

The CD! S will participate in performance improvement and utilization issues in relation to documentation of patient care. S/he will also ensure the accuracy and completeness of clinical information used for measuring and reporting medical staff and facility outcomes. The position will work with all members of the Clinical Documentation Improvement (CDI) program to educate all clinical staff about documentation related issues on an ongoing basis. The CDS is also responsible for the education and training of physicians on how to properly document in accordance with the new ICD-10 standards.

Clinical documentation specialist certification (CDIP certification) through nationally accredited organization (AHIMA or HcPro)

required

;

Registered Nurse (RN) with 5 years of nursing experience, preferably in Med/Surg, Critical Care, Intensive Care or the Emergency Room preferred

1 year of experience with a clinical documentation improvement program! preferred

RHIA, RHIT, CCS, or CPC-H preferred

AHIMA ! or AAPC approved ICD-10 trainer preferred; will require certification after 6 months of hire

Experience in education and training in clinical documentation preferred

Knowledge and involvement in oncologic practice preferred

Utilization Review background preferred.
