RN Case Manager (Home Health)

Location: Muskogee Oklahoma

Description: Angels Care Home Health is presently looking of RN Case Manager (Home Health) right now, this vacancy will be placed in Oklahoma. Angels Care Home Health is currently looking for a full time RN Field Case Manager for our Muskogee, OK Branch location!

Primary Responsibilities:

• Administer skilled nursing care for clients in their place of residence.
• Case manage and provide clinical directions to the interdisciplinary team, patient/ family and referring agency; and assume the responsibility for coordination of care.
• Assure continuity of quality patient care is delivered with appropriate documentation.
• Demonstrate commitment, professional growth and competency.

Requirements:

• Must be a graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Must hold a current Oklahoma State license as a Registered Nurse.
• Must hold a current Oklahoma State Driver License.
• A minimum of 6 months experience as a Registered Nurse in a clinical care setting, home health preferred.
• Good interpersonal skills, proof of current CPR and Hepatitis Profile, reliable transportation with current, valid auto liability insurance.
