Coverage Monday - Friday, 8:00am - 4:30pm - (one week each month Thursday 8:00 am - 6:30 pm and Saturday 8:00am - 2:30pm)

NOTE: Salary is commensurate with education and experience and will be determined by a Nurse Professional Standards Board.

Background Investigation required DUTIES:
The Licensed Practical Nurse works under the direction of an RN and reports to the Primary Care Manager. The LPN is responsible for! observing, recording and reporting changes in the patientÂ's ! condition, as well as participating in the development and modification of the patient-driven holistic care plan for life. The LPN requires general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support Technician & PSA) and Larger Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patient).
BLS Certification is required.

Citizenship: Applicants must be a citizen of the United States.

Licensure: Applicant must have an active, full, current and unrestricted licensure as a Licensed Practical or Vocational Nurse in a State, Territ! ory or Commonwealth (i.e. Puerto Rico), if the United States or the District of Columbia as outlined in VHA Handbook 5005/3 Part II, Chapter III.

Education: The incumbent is a graduate of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing at the time the program was completed.
